Reverse Shoulder Prosthesis for Proximal Humeral Fractures: Primary Treatment vs. Salvage Procedure

Abstract: Background: The optimal treatment for complex proximal humerus fractures (PHFs) lacks consensus, with reverse total shoulder arthroplasty (RTSA) often being a final resort rather than a primary approach. This study aimed to compare outcomes and satisfaction rates of primary RTSA for PHFs versus salvage RTSA for previously unsuccessful treatments. We hypothesized that primary RTSA would yield superior clinical outcomes, functional scores, and patient satisfaction. Methods: A retrospective analysis of RSA procedures between 2011 and 2021 was conducted, focusing on primary RTSA for PHFs or salvage RTSA for failed osteosynthesis. Patients meeting inclusion criteria underwent clinical and radiological follow-up for at least two years. Demographic characteristics, outcomes scores, and range of motion (ROM) were assessed. Results: Of 63 patients, 42 underwent primary RTSA and 21 underwent salvage RTSA. The median follow-up was 50 months. Statistically significant differences favored primary RTSA in forward flexion, abduction, internal rotation, and Constant shoulder score. Patient satisfaction levels did not significantly differ between groups. Complications occurred in 7.15% of primary RTSA cases and 14.28% of salvage RTSA cases. Conclusions: Primary RTSA may yield slightly better outcomes and lower complication rates compared to salvage RTSA. Further prospective studies are necessary to validate these findings. Bone Density Changes at the Origin of the Deltoid Muscle following Reverse Shoulder Arthroplasty

Abstract: Background: Reverse total shoulder arthroplasty (RSA) significantly impacts deltoid length, tension, and structure. Studies have extensively investigated various modifications in deltoid characteristics, such as perfusion, elasticity, caliber, histological changes, and strength post- RSA. However, to date, there is a notable absence of research evaluating changes in bone mineral density (BMD) at the deltoid muscle origin after the RSA procedure. Methods: A retrospective analysis of a consecutive series of RSAs performed between May 2011 and May 2022 was conducted. Inclusion criteria comprised primary RSAs with both preoperative and last follow-up shoulder CT scans and a minimum follow-up of 12 months. Trabecular aenuation measured in Hounsfield units (HU) was calculated using a rapid region-of-interest (ROI) method. BMD analysis involved segmenting three ROIs in both pre- and postoperative CT scans of each patient: the acromion, clavicle, and spine of the scapula. Results: A total of 44 RSAs in 43 patients, comprising 29 women and 14 men, were included in this study. The mean follow-up duration was 49 ± 22.64 months. Significant differences were observed between preoperative and postoperative HU values in all analyzed regions. Specifically, BMD increased in the acromion and spine, while it decreased in the clavicle (p-values 0.0019, <0.0001, and 0.0088, respectively). Conclusions: The modifications in shoulder biomechanics and, consequently, deltoid tension post-implantation result in discernible variations in bone quality within the analyzed regions. This study underscores the importance of thorough preoperative patient planning. By utilizing CT images routinely obtained before reverse shoulder replacement surgery, patients at high risk for fractures of the acromion, clavicle, and scapular spine can be identified.

Reverse total shoulder arthroplasty (RTSA) is known to be a functional rather than an anatomical surgical procedure. In fact, it overthrows the normal glenohumeral anatomy by transferring the convex component to the glenoid side and the concave component to the humeral side. The goal is to allow good shoulder function even in the absence of a functional rotator cuff.

RTSA is based on four cardinal principles: (1) the center of rotation must be fixed, distalized, and medialized at the glenoid surface; (2) the deltoid lever arm must be effective from the onset of movement; (3) the prosthesis must be inherently stable; and (4) the construct must create a semiconstrained joint.

The reverse shoulder arthroplasty conceived by Paul Grammont in 1985 has gradually gained popularity as a treatment for multiple shoulder diseases. Unlike previous reverse shoulder prosthesis characterized by unsatisfactory results and a high glenoid implant failure rate, the Grammont design has immediately shown good clinical outcomes. This semi constrained prosthesis solved the issues of the very first designs by medializing and distalizing the center of rotation with an increased stability of the replacement of the component.The indication was initially limited to cuff tear arthropathy (CTA). It has then been expanded to irreparable massive cuff tears and displaced humeral head fractures. The most frequent problems of this design are a limited postoperative external rotation and scapular notching.Different modifications to the original Grammont design have been proposed with the aim of decreasing the risk of failure and complications and improving the clinical outcomes.Both the position and version/inclination of the glenosphere and the humeral configuration (e.g. neck shaft angle) influence the RSA outcomes.A lateralized glenoid (whether with bone or metal) and a 135  Inlay system configuration leads to a moment arm which is the closest to the native shoulder. Clinical research will focus on implant designs reducing bone adaptations and revision rate, strategies to prevent more effectively infections. Furthermore, there is still room for improvement in terms of better postoperative internal and external rotations and clinical outcomes after RSA implanted for humeral fracture and revision shoulder arthroplasty.

Arthroscopic Treatment of Hill-Sachs Lesion – Chapter of “Shoulder Arthroscopy” June 2023

The Hill-Sachs lesion is a compression fracture of the posterosuperolateral aspect of the humeral head that occurs in association with anteroinferior instability of the glenohumeral joint and might lead to recurrence. The glenoid track refers to the area of contact between the humeral head and glenoid and is defined as 83% of glenoid width. An Hill-Sachs lesion is considered “On-track” or “Off-track” respectively when is inside or outside the glenoid track. With a method based on CT scan the physician might assess whether a lesion is On-track or Off-track. The treatment algorithm frequently used which aims at reducing the risk of anteroinferior shoulder instability takes into consideration either the percentage of glenoid bone loss (greater or not than 25%) and the width and position of the Hills-Sachs lesion (Glenoid track).Those two cut-off values are useful in determining whether the glenoid, humeral or both sides has to be corrected.The remplissage is a safe procedure with a low postoperative recurrence rate, and it allows the surgeon to address large humeral defects. It seems to be safer than Latarjet procedure, due to the fewer overall postoperative complications.

“Surgical management of osteonecrosis of the humeral head: a systematic review.”

— ABSTRACT —

PURPOSE: Osteonecrosis of the humeral head is a rare diagnosis. The aim of this systematic review was to identify published studies and analyses the best clinical evidence available related to the surgical management of osteonecrosis of the humeral head.

METHODS: A systematic electronic search was performed using the PubMed (MEDLINE), EMBASE and Cochrane Library databases. Published studies that reported the outcomes for adult patients treated surgically for osteonecrosis of the humeral head were included. The PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) statement was followed.

RESULTS: Twelve studies were included: five prospective case series and seven retrospective case series. A total of 309 patients, comprising 382 shoulders, were included. Three main surgical procedures were evaluated: core decompression, hemi-arthroplasty and total shoulder arthroplasty.

CONCLUSION: Based on the current available data, core decompression is a safe and effective option for treating low-grade osteonecrosis of the humeral head, while hemi-arthroplasty and total shoulder arthroplasty should be considered for high-grade osteonecrosis. More studies and better-designed trials are needed in order to enrich the evidence and enable researchers to draw stronger conclusions. Since osteonecrosis is an uncommon, though challenging disease, a proper knowledge of its treatment is needed.
